India Healthcare Real Estate Gap Exposes Bed Shortage

India will have to add roughly 2.6 billion sq ft of healthcare real estate by 2035 to match the global average for hospital beds, a property consultancy analysis shows. The country has 1.6 beds per 1,000 people against a worldwide 3.3. Closing that gap means nearly 2.9 million more beds, and it will influence where and how Indian cities expand.

The scale is striking because the existing stock was estimated at about 2.1 billion sq ft in 2025. The required addition is therefore larger than everything built so far, and industry experts say it cannot be met by large multispecialty hospitals alone. Need is also uneven. Comparing chronic-disease prevalence with bed density, the analysis flags Chhattisgarh, Odisha, Madhya Pradesh and Jharkhand as states where beds look scarce relative to illness, suggesting unmet demand for specialist and advanced care. Karnataka, Kerala, Tamil Nadu and Telangana are better served, so their opportunity lies in deepening specialised services. Where beds are scarce, patients usually travel further for treatment, a burden urban planners say falls hardest on low-income households.

Operators and developers are responding with a wider mix of formats: single-speciality hospitals, day-care surgery centres, outpatient clinics, diagnostics, rehabilitation, eye care, fertility and preventive-health outlets. Medical office buildings, which house several providers in one development, and built-to-suit facilities designed around clinical and regulatory needs are also gaining attention. Acquisitions offer another route, giving buyers existing licences and patient networks in crowded urban markets. Delhi-NCR offers an early example. A healthcare venture backed by a listed insurance-marketplace parent has opened a 100-bed multispecialty hospital in DLF Phase 2, Gurugram, with cardiology, neurology, orthopaedics, paediatrics and robotic surgery, plus CT, MRI and laboratory services. It plans 200-250-bed hospitals elsewhere in the region and a franchise model with partner hospitals nationwide. Its stated aim is to link prevention, treatment and recovery at home.

The environmental stakes are easy to overlook. Hospitals run around the clock and use large amounts of energy and water, so the design of this new healthcare real estate will affect city carbon targets and climate resilience. Tier-II and tier-III cities, with larger land parcels and lower land costs, are expected to host much of the building. Planners warn that sites far from public transport can leave patients, staff and visitors dependent on private vehicles. Turning the projected need into viable projects will depend on patient demand, affordability, location and operating efficiency. The test ahead is whether new capacity reaches underserved states and smaller cities instead of clustering in already well-served metros, and whether affordability and low-carbon standards are built in from the start.
